MONTHLY SUBSCRIPTION FEES
The monthly subscription fees cover an unlimited number of office visits, very little wait times at the office, unlimited access to lifestyle assessments and therapy, 24-hour contact with the Office Manager, flexible telemedicine appointments during weekends and holidays.
Individual Membership
In addition to the one time $200 membership registration fee, the base charges for an individual membership are:
Age 18-40: $60/month
Age 41-50: $80/month
Age 51-64: $110/month
Age 65-79: $140/month
Age 80+: $160/month
Please note these fees do not include lab charges.
Family Membership
In addition to the one time $200 membership registration fee, the base charges for an individual membership are:
Family fee is $250/month (up to 4 family members between ages 15-64)
Please note these fees do not include lab charges.
Non-Menshipship Fees
As an alternative to Membership dues, the base fees are:
Initial Office Visit: $300
Established Office Visit: $200
